To submit this form, your Identification Passcode will be required. Earlier, your unique Identification Passcode was mailed to your address. Only after you have activated that code can Story modifications be processed.
"VERIFIED" will appear here if your name and email agree with our record. Otherwise, provide a redacted copy of your current official State identification.
When submitting the story components, a field named State Identification allows for an image file of the redacted ID to be uploaded. This is an example of what we ask to be provided (self-image is optional):
ONLY the name will be published. The address will NOT be published or otherwise shared! The copy must show the validation date and official State identification markings.
The address is used solely to send you via the US Post Office your Identification Passcode. The Identification Passcode serves to authenticate you as the story's active StoryTeller and to confirm your permission for story and profile modifications you initiate occasionally. DO NOT SHARE THIS CODE WITH ANYONE.
Your name, as StoryTeller, will be published within the Story.
You may publish your choice of a self-image; it just must resemble that image on your State identification.
Your name, as submitted, must exactly agree with that on the ID document. Also, if it is not current, the State identification markings are absent, or your street address does not appear, there will be a delay to cure these issues.
If you foresee a problem meeting this requirement, please save your work; before submitting this story, contact your StoryTeller Support Team to open a discussion for a resolution.
There should not be a change here unless there also has been a change to the subject's memorial plaque.
Changes to dates appearing on the memorial also are to be noted.
Such a photo might not be possible due to weather-related decay. Even in that instance, a photo of the memorial enhances the story. Also, if a memorial photo has already been published, but you want to use a different one for the story, you can submit the update here.
If your submitted story already includes a portrait of the subject, the image provided here will replace it. You may include the replaced image in the gallery portion of the story by adding it in the Story Enhancement step (specifically, the section - Video, Audio, and/or Images) found later in this form.
If the story's subject is an individual versus a place or event, identification of nearby relatives' burial sites is encouraged. This image would allow a Visitor at the site to locate subject relatives memorialized nearby.
Other supplemental identifying details, such as the cemetery name, are encouraged. An alternative for the address is a site map/diagram. In that case, state below that a site map/diagram is being provided and upload the file below.
Identify nearby roads if providing a site map/diagram of the memorial's location.
Would you like to change the subject's genealogic record? The existing genealogic record will be replaced entirely.
Provide at least one of these genealogic relationship display options. The subject must be among those included in the display. If the StoryTeller's relationship with the subject is not shown, the StoryTeller will be identified as "Unassociated with the subject." There is no required minimum of inter-relationships.
If you choose to display a genealogic "chart," upload it here. A "Relationship" chart presents a graphical display of one person's relationship to another, including the relationship of each person in between; a "Pedigree" chart displays a person's direct line ancestors; and a "Descendant" chart displays an individual's descendants with the primary individual at the top and the descendants below.
Provide its address; a link to that genealogical record will appear within the Story's Kinship tab.
Are there media to add to the story? As the StoryTeller, the opportunities to enhance your entry are almost boundless. Video, audio, and still-image complimenting the story may be uploaded here. Strive for production quality and concise content. Please, give recognition to the parties who created the recording. (Peek·into·the·Past offers professional narrating, editing, and video and audio production.) All voices and faces must be identified with first and last names.
Are there videos, audio, images, sketches, or drawings currently part of the story which you want to be removed? Give their description here; otherwise, state "None." For modifications to the media now part of the story, could you describe it here and add the revised media to the previous field?

The narrative is a terrific way to pull story components together, making them easily relatable. Visitors particularly enjoy StoryTeller's insights and reflections. A narrative format is ideally suited for sharing those; it allows for the free-form expression of the StoryTeller's passion for the subject. Below are two ways to provide a narrative - write it in the text field or prepare it as a separate document and upload the file.
A distinct section of the Story format is "Tidbits." Here is a suitable place to point out defining moments that occurred during the Subject's life; those often impact a Visitor's perspective of the Story. Tidbits may include anything you feel expands on the Story narrative but may best be separated. Visitors' comments regarding the Story will be posted within the Tidbits section.

This self-image chosen to be published must resemble that on the submitted State identification; therefore, that ID photo cannot have been among the redacted identifying information.
The official State identification in our records must show its image of you, and the self-image being provided here must resemble that image. If the most recent identification provided to us has the image redacted, an update showing your image along with your name and address (and having redacted all other personal details) can be dropped here, too.
Peek·into·the·Past strives to ensure Visitors can contact the StoryTeller. The Administrator will relay all Visitor communication directed to the StoryTeller; Administrator will neither reveal the StoryTeller's nor a Visitor's email address. A USPS letter requesting acknowledgment of ownership of the provided email address will be mailed to the StoryTeller. Once we receive validation, the address will be activated within your profile.

"VERIFIED" will appear if details in the fields "Story title," "Your Name as in the story," and "Your identification passcode" match your Profile Record.
Without a passcode, refer to the Visitor Portal for clarification regarding the process. There, your question can be initiated for us to take prompt action.
Before invoicing, the Invoice Total will be reduced, first by any allowance due from having paid a subscription, then by the unredeemed value of your E-Gift Card. To the extent the unredeemed value exceeds the Total, you will be issued a replacement E-Gift card with the original expiration date.
